Follow these directions:
- 1) Fill out the forms completely
- 2) Make copies for each Bureau (Transunion, Experian and Equifax)
- 3) Make copies for each creditor that you believe was a false account.
- 4) KEEP A COMPLETE COPY OF EVERYTHING FOR YOURSELF
- 5) Mail each Bureau and all Creditors a copy
- 6) Include a copy of your ID Kit (this is your ID, Drivers License or Passport, copy of a piece of mail with your name and current address, copy of your social security card)
